Our name reflects how we work in two connected ways. First
we do not ignore the elephant in the room—issues that are politically sensitive, uncomfortable, or often left unspoken in policy and organizational processes. We help our partners surface these realities constructively and address them directly.
Second, we focus on seeing the whole system
Complex policy and management challenges cannot be understood by examining isolated components. We take a holistic, systems-based perspective to understand how structures, actors, incentives, and dynamics interact over time.
These two principles reflect who we are: the courage to face the truth and the prudence to understand the complexities of systems.
CEO and Co-Founder Note
Through our work in health systems and health policy development—across research, teaching, and collaboration with policy institutions—we have repeatedly encountered a central challenge: many of the most critical issues in health systems are complex, interconnected, and not amenable to linear solutions.
Experience from health systems and health policy research has shown that policy responses developed without a systemic perspective often struggle to achieve sustainable gains in efficiency, equity, and resilience. Moreover, some of the issues with the greatest impact on system performance are politically sensitive or institutionally difficult to address—the “elephants in the room” that are widely recognized but rarely examined openly.
BigElephant Advisory was founded on the belief that effective policy and management, particularly health system reforms, require both the courage to confront reality and the discipline to understand the system as a whole. Our work is grounded in systems thinking and emphasizes co-creation through Policy Labs, which bring together policymakers, researchers, practitioners, and stakeholders to jointly explore complex problems and develop policy options that are analytically sound and institutionally feasible.
Our role is not to offer ready-made solutions, but to work alongside policy actors to strengthen shared understanding and co-create approaches that can be implemented in real-world contexts.
We hope BigElephant Advisory can serve as a bridge between research and policy, supporting long-term transformation toward more equitable and sustainable health systems.
